All students must have a physical examination annually:
Physical examination taken April 1 and thereafter is valid for the following school year; physical examination taken before April 1 is valid only for the remainder of that school year.
Athletes WILL NOT be allowed to participate without having a current physical exam on file.
All documents in regards to the Athletic Physical should be submitted to the School's Activity Department. You can scan and upload your completed Physical Examination card below. If that is not possible: please send, drop off or bring your completed Physical Examination card with you to your Athletic Sports Meeting.
